Claim your GSU premium!
Unit 1 members: Follow the link to find out how you can claim your GSU premium under your Health Care Spending Account. For those who opted-in family members to the GSU plan - receipts will be available for the family portion on Monday, March 1st in the GSU office.
Follow these simple instructions in order to claim the premium you paid to the GSU for your health/dental coverage. If you are a Unit 1 member with over 50 hours of work from September 2009-August 2010 you have a $800 HCSA with $300 reserved for dental. More information is on this website under the Benefits link.
HOW TO CLAIM YOUR GSU PREMIUM TO YOUR CUPE 3902 UNIT 1 HCSA
1. Go here: https://www.rosi.utoronto.ca/
2. Click “Login” and log in.
3. Go to “Financial Accounts”.
4. Click “Show Invoice” and PRINT.
5. Click “Show Detailed Report” and PRINT.
6. Collect BOTH printouts.
7. Fill out Unit 1 “Spending Account & Enrollment Claim Form” (available in the office or online).
8. Send the form AND the printouts to the address on the form.
9. Get your money!
