Dental Plan

If you are a registered UofT student or a post-doctoral fellow (or were at the time you were hired as a TA, student instructor, lab demonstrator, etc) and you WILL work a minimum of 50 hours in the bargaining unit between September 1, 2007 and August 31, 2008, you are entitled to a $500 dental rebate. You can go to the dentist at any point during this time period, even after you have stopped working!

Your claim should include an account statement (for work done) and receipt (with total paid) for dental work and a completed Dental Rebate Request Form for 2007-2008.

Bring or mail your claim in to the Union office (180 Bloor West, 9th floor) or to the UofT Benefits Office (215 Huron St, 8th floor, Toronto, M5S 1A2), or, at UTM, to Business Services, North Building, Room 157, and at UTSC to the Personnel Office, Room S418C).

Claims are processed at the end of each month, along with payroll, so if you can submit your claim to the Union office by the 8th of any month, you can expect your reimbursement at the end of that same month, either as a separate payment, or along with any pay you're receiving that month (in the latter case, it will be identified as a separate line item on your paystub, called "dental rebate"). If you submit your claim after the 8th of any month, you can expect to receive your rebate at the end of the following month.

The deadline to claim your Dental Rebate entitlement for the 2006-2007 academic year has now passed.

Spousal/child coverage

All Unit 1 members with at least 50 hours of appointment can also get dental coverage for their spouses (married or common-law) and children under 21. NEW AMOUNTS: Each spouse is entitled to up to $500 as well, and each dependent (i.e., child) is entitled to up to $400 worth of dental rebates.

Please note, that if you and your spouse are both members of the bargaining unit and therefore both entitled to a dental rebate, each person only gets his/her personal amount, NOT the spousal amount as well.

Other plans

IMPORTANT DENTAL PLAN INFORMATION FOR MEMBERS COVERED BY ANOTHER PLAN

The new Unit 1 Collective Agreement contains an important new provision that extends the dental and health care spending accounts to persons who may already have other, non-student, coverage.  The old contract restricted access to the dental plan to persons without other non-student coverage.  Many members are still under the impression that the old provisions are still in force.

What does this mean?

It means that if you are currently covered by a spousal dental plan, you can claim dental expenses not reimbursed by that plan from the CUPE dental plan. However, your spouse would not be eligible for any reimbursement from the CUPE plan. However, your children would be able to claim from both plans. The same applies to the Health Care Spending Account (a new benefit in this Collective Agreement).

How do I do this?

Claim first from your primary plan, then claim the balance from the CUPE plan.

If you are denied coverage for any reason whatsoever, please contact the Union. We can solve problems either informally (through discussion with Labour Relations or Benefits) or through a formal grievance process. Most problems can be solved informally.

If you are a UofT graduate student, you now ALSO have dental coverage through the Graduate Students’ Union, in addition to the CUPE dental rebate. You can combine these benefits for coverage up to $1,000 (or you can choose to opt out of the GSU dental plan and get your premium back – it all depends on what you think your expenses will be).

Opting out of the GSU dental plan

To opt out of the GSU dental plan, you can now use the GSU electronic opt-out application: they have set up a dummy Green Shield "policy" for our rebate, to allow that field in the electronic form to function, even though our dental rebate does not really have a policy number, and is not really handled by Green Shield. Nevertheless, to opt-out on line, please use the following information:

Subscriber ID: CUPE 3902

Policy#: 30131

Underwriter: Green Shield

The opt-out period for 2007 has now passed. For more information, please contact the GSU at dental.gsu@utoronto.ca or 416.978.8465.
